Severe forms of endometriosis that do not respond to hormonal treatment require surgical treatmentUnfortunately, women of all ages are susceptible to this disease, including very young patients, women of reproductive age, and postmenopausal patients.In severe forms, not only the female reproductive organs are affected, but also the urinary system and gastrointestinal tract.Between 2010 and 2025, the surgical department performed surgical treatment on 4,809 patients with various forms of endometriosis (N80, N80.1, N80.2, N80.3, N80.4, N80.5, ICD-10 N80.6, N80.8, N80.9). Of these, bowel surgery (bowel shaving, discoid bowel resection, segmental bowel resection) was performed on 1,439 patients. Over the past 10 years, the number of patients undergoing surgery has increased annually.

We have reduced the complication rate after surgical treatment of endometriosis to 11%.In our practice, we consider all types of postoperative complications, using the CDC scale for assessment. The most important (significant) surgical complications according to this scale begin with Grade IIIa.According to our data, in patients with colorectal endometriosis after instrumental bowel resection, anastomotic leakage occurred in 6.6% of cases, rectovaginal fistula in 3.8%, and hydronephrosis in 4.5%.Various complications after surgical treatment of severe forms of endometriosis require a high level of surgical training. The key to successful surgical treatment is readiness to operate in any situation: on the reproductive system, the urinary system, and the intestine at various levels.Based on our experience, we have developed algorithms for identifying complications aimed at reducing the time from the diagnosis of the accident to surgical treatment.
