Title: Effectiveness of Platelet rich plasma versus Granulocyte Colony Stimulating factor to improve endometrial thickness in women undergoing infertility treatment

Abstract

Background: IVF is one of the major management options for infertile couple and its outcome is based on various factors, endometrial thickness is one of them. In order to achieve successful outcome of IVF treatment it is essential to have responsive endometrium. Recently intrauterine instillation of platelet rich plasma (PRP) and granulocyte colony stimulating factor (G-CSF) have evolved as promising therapeutic options in such cases. But there is no proven evidence to prefer one modality over the other.

Methodology: A cross sectional analytical study was conducted on 50 patients with or without failed IVF cycles and having thin endometrium on TVS. Patients were randomly divided in two groups: group A and B. Group A (n=25) were treated with intrauterine PRP infusion for endometrial thickness improvement prior to embryo transfer. Other group- group B (n=25) were given intrauterine infusion of injection GCSF (300mcg) on the day of trigger or day 11 of cycle along with post embryo transfer, 60 units of G-CSF injection subcutaneously. The changes in the endometrial thickness during embryo transfer and the pregnancy outcomes in the two groups were compared.

Results: A statistically significant increase in endometrial thickness was seen with G-CSF group as compared to PRP group (p < 0.01). Whereas on comparing the two modalities with respect to improvement in the pregnancy outcomes no significant difference was noted in the two groups.

Conclusion: For the management of thin endometrium G-CSF was found superior to PRP as treatment options. Both groups showed no significant differences in the pregnancy outcomes. PRP is readily available and cost-effective therapeutic option in managing cases of thin endometrium. Further larger studies and RCTs are needed to prove the effectiveness of these treatment modalities.
