Title: Determinants of Late Antenatal Care Attendance Among High Parity Women in Uganda

Abstract

Timely and adequate Antenatal Care (ANC) is vital in preventing adverse pregnancy outcomes and reducing maternal and neonatal mortality. High-parity women (those with five or more children) face a high risk of maternal mortality. Yet, insufficient information on late ANC timing in this group hinders Uganda's efforts to improve maternal and child survival. This study aimed to identify factors associated with delayed initiation of the first ANC visit after 12 weeks of gestation among high-parity women in Uganda.

Utilizing nationally representative data from the 2016 Uganda Demographic and Health Survey, the study included 5266 women aged 15–49 with five or more children. Employing a complementary log-log regression model, the research revealed that 73% of high-parity women delayed their first ANC visit. Late ANC attendance was associated with proximity to health facilities, partner cohabitation, partner's education, previous delivery in a health facility, and the desire for more children.

The study suggests promoting and reinforcing health facility deliveries to enhance timely attendance and maternal survival among high-parity women. Integrating family planning services with ANC and postnatal care education is recommended to encourage early ANC visits within the recommended first trimester. The study also advocates for increased support for education, sensitization, and advocacy programs, specifically focusing on health facility-based deliveries. Strengthening Volunteer Health Team (VHT) activities and community engagement initiatives are highlighted as potential avenues for achieving this.

Godfrey Tumwizere is a demographer with research interest in Adolescent and Maternal Health, Family planning, and Sexual Reproductive Health and rights. He is a graduate of the Master of Science in Population and Reproductive Health of Makerere University, Kampala, Uganda. He works with Action 4 Health Uganda as a Technical Specialist-Sexual Reproductive Health and Rights.
