Home Blood Pressure Monitoring for Maternity Patients
Abstract
I am an Advanced Midwife
Practitioner who specialises in caring for women with Hypertensive Disorders in
pregnancy and postnatally. I introduced a home blood pressure monitoring system
in Our Lady of Lourdes Hospital in Drogheda in June 2024 and to date this has
proven to be extremely successful. This a Department of Health funded virtual
health seedling initiative and have been revolutionary in the way we manage our
Chronic and Gestational Hypertension obstetric patients. The system I use gives
instant feedback from the monitor, eliminating user error and patient bias. The
antenatal patients also carry out a weekly urinalysis for the detection of
protein, to identify preeclampsia at an early stage and upload the results onto
an app this is then immediately available to the clinician in the hospital. I
carry out the vast majority of care virtually and review on an “as required”
basis. I am also in the process of setting up a hypertensive clinic which will
care for patients availing of Home Blood Pressure Monitoring in Maternity from
referral to the clinic until 6 weeks postnatal. At a 6 week postnatal discharge
visit, women will be offered contraception and if they wish, there is a link
between myself and the ANP in the Ambulatory Gynaecology that will facilitate
insertion of Intrauterine Contraception Devices for long term contraception on
the same day as their discharge appointment. This allows hypertension disorders
to be optimised prior to conception in the future. We had reduced our inpatient
admissions for hypertensive disorders from 68.3% to 9.9%, we had reduced our
outpatient postnatal appointments from an average of 6 per patient to 2 per
patient, which in turn has reduced our carbon footprint by over 3000km
Biography
Kelly began her midwifery training in 2009
at the University of Southampton. After she qualified in 2013, she worked in
the UK for a short period before returning to Ireland. She commenced working in
the Coombe Hospital in 2014. She began working at Our Lady of Lourdes Hospital
in Drogheda in 2019. In 2022, Kelly began her cAMP role. Most recently she has
completed her Postgraduate Certificate in Advanced Practice (Midwifery); and an
Ultrasound Scanning for Midwives module to ensure continuity of care for the
women within her caseload and to reduce patient wait times. Kelly is now a
registered AMP in Specialised Care Pathway. Her caseload includes women with
hypertensive disorders, she has developed a system for Home Blood Pressure
Monitoring in Pregnancy which is proving to be extremely successful.
