Abstract
Background: Vallecular
cyst is a rare but life threatening airway anomaly that needs prompt diagnosis
and surgical intervention. It presents with sridor and varying degrees of
discomfort with failure to thrive. Coblation assisted marsupalisation proves to
be an effective and safe surgical technique in the management of congenital
vallecular cyst in neonates.
Case
presentation: Here we present a 45 day old female baby with stridor, chest
in-drawing and failure to thrive. Detailed evaluation with laryngoscopy and
radiology confirmed the diagnosis of congenital vallecular cyst. Coblation
assisted marsupalisation was performed. Post op period and follow up showed
marked improvement and complete disappearance of stridor as early as day 5.
Conclusion: Though
rare, congenital vallecular cyst is a life threatening condition that can be
rectified completely by prompt intervention. Direct visualisation of the airway
is the most important tool in diagnosis. Coblation technique proves to be a
promising technique in marsupalisation of the cyst wall with minimal bleeding
and post operative edema. This itself helps in speedy recovery and resolution
of symptoms. Smaller cysts can be managed even without tracheostomy and earlier
extubation.

Dr Lakshmi Venkitaraman, is
one among the 6 specialist surgeons in the Department of ENT, in 700 bedded
government aided hospital in Ernakulam city. She has completed her specialty
training at the age of 28 years from Manipal Hospital. She has shown special
interest in paediatric airway and related problems and has been trained in an
institution doing pioneer work in the field.
