Title: Evaluation of the Accuracy of Implementation of Obstetric Referral Cases to the Emergency in the National Health Insurance (JKN) Era

Abstract

To investigate the circumstances of obstetric referral cases at the emergency department of Dr. Cipto Mangunkusumo Hospital, as well as the accuracy of referral implementation. This is a cross-sectional study. Medical records of obstetric patients at the Emergency Department of RSCM from January 2013 to July 2014 were obtained, and information about patients and their characteristics were collected. The accuracy of referral cases is based on emergency obstetric criteria according to the BPJS rules. Primary healthcare has become the leading referrer. Inappropriate diagnostic referral cases amounted to 21.2% when JKN was first implemented in 2014. Cases with improper diagnoses were more often referred when JKN was implemented, amounting to 16.8% and 6.9% of non-emergency referral cases in 2014. Referral implementation has not been efficient or effective, and the number of obstetric referral cases to the emergency department of RSCM that can be handled in secondary advanced health services after the application of JKN is still high. Monitoring and evaluation are needed by the Local Health Office to improve the quality of the referral system applied in the new JKN era. 

Biography

Omo Abdul Madjid, M.D., Ph.D. is an esteemed figure in obstetrics and gynecology in Indonesia. He completed his Doctorate in Medicine from the Faculty of Medicine, Universitas Indonesia, in 2018, following a master’s degree in Hospital Management from Universitas Gajah Mada in 2017. His medical journey began with a specialization in Obstetrics and Gynecology from FKUI in 1993. Dr. Omo's professional trajectory was rich in significant appointments. Notably, he served as the Director of the RSAB Harapan Kita and the General Director of the RSCM. His commitment to medical education is evident through roles as principal clinical educators and coordinators of training at various esteemed institutions. He also held leadership positions in numerous medical associations, including chairmanship roles. Additionally, Dr. Omo contributed to the medical community as an editor for the Indonesian Journal of Obstetrics and Gynecology and as a Master Trainer for various programs. His dedication to advancing healthcare quality is further exemplified by his current role as the Chairman of the Audit Medical Division.
